Name of the Applicant - Name of the person in whose name the vehicle will be booked (not necessarily the person who makes the payment)

Pan Number - PAN Card number of the applicant in whose name the vehicle is booked. In case, you don’t have a PAN Card please fill form 60 or 61 and tick the relevant box. Either of the 3 is mandatory.

Address Fields Mobile no., Landline, Email ID – it is very important to fill these fields correctly, as these will be referred for future communication, SMS intimations, mails etc relating to allotment and refund.
